We aim to give support to people with a learning disability and their carers by assessing and managing care needs and providing information and advice. • Giving appropriate advice and information.• Assessment of needs for service users (Social Care and Wellbeing assessment) and their carers (Careers Assessment).• Assess and work towards personal outcomes with the service user.• Arranging respite or long-term accommodation and community support packages based on assed needs and personal outcomes. • Reviewing and reassessing current care provision.• Assessing and intervening with regard to support to manage risk including safeguarding enquiries.• Working in a person centred and strengths based way.
We provide support for adults with a learning disability living in the community, at home or in supported housing and their families or carers.
