What we do

This is a short term service designed to support those due to be discharged or recently discharged from hospital providing relevant information and practical support, such as shopping, prescription collection etc. We are also able to signpost or refer to appropriate organisations should you need further support.

Who do we support

Available to anyone aged 18 + who requires support for a limited time to support their recovery and wellbeing where there is no alternative support.

Referral by a relevant health or Social care Professional including through the Social Care Single Point of Access in the relevant county
