VCRS is an integrated Health and Social Care service run by Cardiff & Vale University Health Board and Vale of Glamorgan Social Services. Our team is based in Barry Hospital and consists of various social care and health professionals.We offer a free period of assessment (max 6 weeks) to enable you to come home from hospital with the support you need. We also support people at home to prevent an unnecessary admission into hospital. Following an assessment of your needs and goals, we will work with you to develop a reablement programme to maximise your independence. This may involve some support with everyday tasks such as personal care, making meals, domestic tasks and shopping. We do things ‘with’ you and not ‘for’ you to build your skills and confidence.You may be assessed as benefiting from some therapy input from one of our team which may include some exercises, practicing everyday skills or some nutritional advice.
We support people who are over 18 years of age who live in the Vale of Glamorgan.
Depends - We provide a free period of assessment. If long term support needs are identified the service may be chargeable subject to a financial assessment (maximum charge is £90 per week)
All referrals come to VCRS via Contact One Vale (C1V). Referral is usually made by your Hospital Ward or GP. C1V can be contacted on 01446700111 for advice on referral to our service
