What we do

Enabling re-settlement from hospital by providing up to 12 weeks of support depending on the level of help required. The support we can offer is varied from practical to emotional support including:
- Door-to-door transport for essential health- care journeys Help with practical solutions (for example, picking up or arranging for a prescription delivery or food shopping)
- Health and wellbeing support (including emotional wellbeing)
- Help to rebuild your confidence in order to live a more independent lifestyle
- Help with administrative tasks such as DWP checks or tenancy concerns.

Who do we support

Patients aged 18+ who have been unable to leave hospital due to housing issues. We aim to help them address these issues and enable them to live independently within their own home and avoid homelesssness

This service is for people aged 18+.

Can anyone use this service?

We support patients aged 18+ referred from hospitals in the Abertawe Bro Morganwg University Health Trust.
