Purpose: Reduce hospital discharge delays and promote independence following a hospital stay. Who we help: Adults aged 18+ living in the Swansea area who are clinically fit to return home. Support can be provided for up to 12 weeks at home.Goals and impact: Enabling timely hospital discharge, improving quality of life, reducing pressure on the health system through independent livingActivities can include:Re-settlement and safeguardingHousing supportHelp with administrative tasks such as DWP checks or tenancy concerns.Socialisation and emotional supportHelp rebuilding your confidence to live an independent lifestyle.Please be aware we cannot offer befriending services, transportation or handle medication.
Patients aged 18+ who have been unable to leave hospital due to housing issues. We aim to help them address these issues and enable them to live independently within their own home and avoid homelesssness
