Have choice and take control
We are an integrated health and social care team providing a service to people with mental health issues who live in the Vale of Glamorgan. We can:• Provide individuals with care and support within their own home• Arrange care within a nursing, residential setting• Facilitate hospital discharge• Arrange urgent admission to hospital• Review and reassess current care provision• Assess and intervene in situations where an adult with care and support needs may need safeguarding• Provide advice and support to carers and families• Signpost to other agencies and professionals
Our Vale Locality Team offers a service to people with mental ill health between the ages of 18 and 65 living in the Vale of Glamorgan, or you are a carer supporting an adult or adults with care and support needs.
Depends - You may be asked to pay toward the cost of any Care and Support that you receive dependent upon a Financial Assessment. You will be informed of all charges throughout the process.
Referral is usually via the GP. We also accept self-referrals if people have been open to a VLT within the last 3 years.
