What we do

The Vale of Glamorgan Integrated Discharge Service is part of Adult Services and is a team of social workers, based at Llandough University Hospital of Wales, UHW, Princess of Wales and Sam Davies Ward at Barry Hospital, to support the rapid and safe hospital discharge of people who will require care and support after their hospital stay.

The social worker will provide or signpost to suitable Information, Assistance and Advice that would be helpful to people after their discharge.

This could involve providing or facilitating the provision of care and support such as domiciliary care, day services, respite as well as residential and nursing care.

This service is for people aged 18+.
