We aim to give support to the service user and their carers by assessing and managing care needs and providing information and advice. • Giving appropriate advice and information.• Assessment of needs for service users and their carers.• Support to create a care plan which may include targeted support to meet identified outcomes.• Arranging referrals to other agencies or professionals e.g. health professionals, Carers officer. • Arranging for respite or longer term accommodation.• Reviewing and reassessing current care provision.• Assessing and intervening with regard to support to manage risk.
We provide support for adults with a learning disability living in the community, at home or in supported housing and their families or carers.
