What we do

This is a short term service designed to support those due to be discharged or recently discharged from hospital providing relevant information and practical support.

Who do we support

Available to anyone aged 18 + who requires support for a limited time (up to 2 weeks) to support their recovery and well-being where there is no alternative support. Signposting to sources of help and offer of shopping if needed.

This service is for people aged 18+.

Referral by a relevant health or social care professional including through the Social Care Single Point of Access in the relevant county
