We offer a social prescribing service to support people to access activities they can enjoy in their local community that improves health and well-being. The benefits are:- learn new skills or participate in a new activity- improve mental, emotional and physical health and well-being- meet new people and make new friends- receive the right practical, therapeutic and social support from others- become empowered and increase self confidence and self esteem- becoming involved people often become resource for others and the local community.
The service is for anyone over the age of 18. The service is for patients who need support to improve their health and well-being or feel isolated, stressed or unsure where to get help.
