What we do

The Adult Single Point of Access Team (known as SPoA) are the first point of contact for Wrexham County Borough Council’s Adult Social Care services and receive referrals via Contact Us, Wrexham CBC, from individuals, their families, carers or friends and other professionals including North Wales Police, Ambulance Service, Fire Service, Doctors, Hospitals, District Nurses, Housing etc.
We will undertake an assessment by having a ‘What Matters’ conversation with you, usually over the telephone, where we will talk to you about your needs, what is important to you, and where you might be struggling and help you identify solutions within the community through information, advice or assistance. Following a 'What Matters' conversation with you, we may make a referral to a specialist team within the local authority for a full care needs assessment, however this will be discussed and explained to you fully should this be the case.
