What we do

Homecare and Reablement is a 24 hour service committed to providing quality person-centred care that supports people to live independently in their own homes.
We develop individual and flexible support packages following an assessment of need.
We can provide care, support and rehabilitation following discharge from hospital.
We work with people to develop and maintain friendships and support integration into the local community.
The service supports people living in extra care housing in Wrexham: Maes-Y-Dderwen and Plas Telford.

Who do we support

Adults who require assistance with activities of daily living, such as personal care, physical and emotional well-being, maintaining nutrition or meal preparation.
The service supports individuals with physical disabilities, learning disabilities and sensory impairment, frailty associated with age, and people with mental health needs.
The service also offers short term care to children and young people to support them to live with their parents/guardians in the family home.

This service is for people of all ages.

Is there a charge to use this service?

Depends - No cost for up to 6 weeks.
For long term support, a financial assessment will be offered, as there will be a charge based on individual financial circumstances. Maximum charge per week is currently £100.

Can anyone use this service?

Yes. Contact 01978 292 066 who will make the necessary referrals for an assessment, if this is required. Other support may be offered at this time.

Monday – Thursday: 8:30-17:00
Friday: 8:30-16:30
