Age Connects North Wales Central Conwy Community Navigator supports social prescribing. We work alongside the local authority's Single Point of Access Conwy and are embedded within the local authority locality teams.Our Community Navigator will conduct a What Matters assessment to determine what help we can provide and signpost you to suitable support in the community.
The service is for adults of all ages that need additional support with non-medical issues such as, isolation, loneliness and loss of motivation and confidence.
This service is for people aged 18+.
