What we do

Following initial re-settlement from hospital discharge we may provide up to 3 weeks of support (more if needed) depending on the level of help required. The support offered is varied from practical to emotional support including; transport for essential healthcare journeys, shopping, prescription collection, emotional support, confidence building, arranging bill payments, setting up Careline, short term use of mobility aids.

Who do we support

Adults who have experienced an episode of care in hospital and who live on their own and/or with their family or a carer who is unable to provide the necessary assistance. Adult in–patient users from acute community hospitals who live in Mid Wales and the need for low level support has been identified.

This service is for people aged 18+.

Is there a charge to use this service?

No

Can anyone use this service?

DLN’s, OT’s, Ward staff, G.P’s, other health care professionals, community connectors as well as self and family.
