We support clients aged 50+ who have recently visited or had a stay in hospital, or those who are being managed on the community following a period of ill health or trauma. We're here to help you get back on your feet. We provide holistic support that is patient lead for up to 6 weeks. We can support with resettling, well-being, shopping needs, prescription pick ups, medical correspondence and much more.
