What we do

We aim to provide adults and their carers with the right support at the right time by the right person.

Our Common Access Point team has Access and Information Assistants, who will talk to you about your situation and provide you with information, advice and signposting. The Access and Information Assistants can also put you in touch with community based services, such as local.

If we can't help with information, advice or signposting then they may discuss your circumstances with our Multi-Disciplinary Team to see if they can offer any specialist advice or if they can help.

Who do we support

Adults and their carers. This team deals with all initial enquiries and referrals relating to older people and adults with a physical disability.

Please note that we are currently experiencing an unprecedented volume of calls, but we will try and respond to your enquiry as soon as possible.

This service is for people aged 18+.

Is there a charge to use this service?

Depends - Information and advice is free. Some services provided may be subject to a charge, or the Council may ask people to pay towards their care and support (subject to a financial assessment).

Can anyone use this service?

Anyone can contact the Common Access Point to access information and advice on Adult health and social care.
